REFERRAL CARDS ARE AVAILABLE AT THE FRONT DESK AT THE CHRONIC HEALTH CLUB

1.)  Put your name on a referral card and give it to anyone who might be interested in joining The Chronic Health Club.

2.)  When they present the referral card with your name on it when signing up in person at the club they will receive their second month of membership free.**

3.)  After the new member's payment for their first month of membership has been processed, staff at the club will adjust the referring member’s next month’s scheduled autopay to $0.00.***

Everyone wins!

PROGRAM TERMS AND CONDITIONS

** In order to receive their second month of membership free, the new member must sign up for a paid month to month contract and provide billing and payment information at the time of sign-up so that their first month of membership can be charged at the time of sign-up.

*** The referring member's automatic payment adjustment for a free month of membership will not be processed until after the new member's payment for their first month of membership has been successfully processed.

Example:  John gives Jane a referral card with his name written on it.  Jane brings the card into The Chronic Health Club with her to sign up for a membership and signs up for a month to month contract with her second month free.  Jane pays for her first month of membership at sign-up and her second month’s payment is set to $0.00 by the staff.  Once Jane's first payment on her membership is processed, John's next scheduled automatic payment is reduced to $0.00 by the staff.

If John refers more than one new member, John receives a free month off of his membership for each new member he refers after their first month’s payment is processed.

Referrals of new members participating in an insurance program membership such as Silver Sneakers, Prime, Renew Active, or AARP At Your Best do not qualify for credit in the referral program, however, a member participating in one of these insurance programs CAN refer a new paying single or family membership member and the new member will still receive their second month free.

Example #1:  John has a family membership at The Chronic Health Club.  John refers Jane who signs up for a free Silver Sneakers Program membership through her insurance company.  Since Jane is participating in an insurance program, John cannot receive a free month off of his automatic payment schedule for this referral.

Example #2:  John has a free Silver Sneakers Program membership at The Chronic Health Club as a benefit through his insurance company.  John refers Jane to the club who would like to sign up for a family membership at the club.  Jane brings the referral card into The Chronic Health Club with her to sign up for a membership and signs up for a month to month contract with her second month free.  After Jane’s payment for her first month of membership has been processed, the staff adjusts her second month’s payment to $0.00.

In the event that a referral scenario not outlined in this document arises, The Chronic Health Club and its staff reserve the right evaluate each scenario on a case by case basis and to approve or deny referral credits at any time for any reason.

Referral credits to current members are subject to the following rules and limitations:

1.) Credit to a current member for a referral will only be issued once the new member’s payment for their first month of membership has been successfully processed.

2.) Current members receive one free month of credit on their autopay schedule for each membership contract that is activated (regardless of how many actual members are party to the contract for membership) and has at least one automatic payment completed on the contract.

3.) Automatic payment credits for current members as a result of a referral will be given on the month following successful receipt of payment for the newly referred member's first contract payment.

4.) If a current member accumulates credit for multiple new referrals, the staff of the club make its best effort to track credits accrued and make adjustments to the current member's automatic payment schedule for each successive month until the credits are used up.

5.) Training membership clients do not qualify for a full free month's credit for each referral. Instead, if a current member is a training membership client, they will receive a credit on their next month's training membership fee equal to the monthly membership fee of the newly referred member's month to month contract fee.

The tracking of credits is a manual process and may be subject to human error. Each member assumes responsibility for following up with the staff at the club if they feel a credit has not been issued correctly.

The Chronic Health Club and its staff reserves the right to deny credits, change the terms or terminate the referral program at any time without notice.
